How Do I.... Register With The Practice?
You can collect the relevant paperwork from our reception desk. Please fill in all paperwork with as much information as possible and bring it back to the surgery reception desk. Please ensure you give us your NHS number; this is on your medical card or you can phone your previous GP to ask for this number. To confirm residency, we will need to see a copy of a utility bill or bank statement and this will be copied and scanned into your patient record.
When you return your forms to us an appointment to register will be given.
Please avoid our busiest period, 8.30am to 12.30pm, when you return your paperwork.
This will allow our reception staff to give you their full attention.
All patients
16 years old and over will be seen by our practice nurse for a routine health
check. This enables us to have a base line of your general health which we can
check against should you become ill. The appointment lasts for 10 minutes. After
your health check, your paperwork will be reviewed by a GP who will confirm your
suitability for registration. Then your information will be entered onto our
computer system. This process can take up to four days before seeing a doctor
or nurse. It is best to telephone the appointment line to make your appointment
and they will check if you have been registered.
You will be registered with
Bellegrove Surgery but you may express a preference for a specific doctor, if
they are on duty. However, on a Thursday morning, to prevent a long wait for
a preferred doctor, we do appreciate it if patients agree to being seen by any
doctor.
