How Do I.... Register With The Practice?

New patients must attend for a consultation and health check with the nurse. To assist us, we ask that a simple questionnaire be completed to acquaint us with any relevant medical history and a current medication form be filled out for our repeat prescribing system.

Please bring your medical card. You will be registered with Bellegrove Surgery but you may express a preference for a specific doctor, if they are on duty. We aim to provide a high standard of family medicine and provide a full range of services.
